CSK Certificate

Hi all
Does anyone have a decent scan of a CSK certificate that they'd be willing to email to me, so I can see what I need to try to get hold of from LR?
Would be nice to see what it looks like and maybe photoshop it to suit my car's number until I can get hold of the real thing - if thats still possible!
